This tool is designed to help guide a selected team to brainstorm and determine the root causes of identified slip, trip and fall (STF) hazards. The team should have a leader who has knowledge of STF and other members such as workers, supervisors or managers, joint health and safety committee (JHSC) member or health and safety representative (HSR), and health and safety specialists, if any.
This root cause analysis (RCA) tool helps the team systematically consider different aspects of the hazardous condition or situation and what the root cause(s) are of the specified STF hazard.
